A SAMHSA sponsored webinar developed under contract by the National Council for Behavioral Health, will take place Wednesday, April 18, at 12:30 p.m. – 2:00 p.m.
Peer operated services can help people with more extensive health, behavioral health and social needs to avert or better manage crisis, prevent relapses and readmissions, and offer a way out of homelessness and re-incarceration. This presentation will offer a deeper dive into how two such models operate. Hands Across Long Island has an impressive record in effectively engaging homeless individuals and in promoting successful re-entry from prison. The New York Association of Psychiatric Rehabilitation Services created the peer bridger model to engage and assist thousands of New Yorkers to successfully transition from state and local hospitals into the community and to help reduce avoidable use of emergency rooms and inpatient services.
